• Please arrive 15 minutes before your OPD slot at Counter 1 or 2.
• Bring your previous doctor prescriptions, lab reports, and current medicine strips.
• One OPD registration consultation slip is valid for 7 days (1 free follow-up with same doctor).
• For fasting blood tests (Sugar / Lipid / Thyroid), come on an empty stomach (8–10 hrs water only).
• Visiting Hours: Evening 4:30 PM – 6:30 PM. ICU Briefing: 11:00 AM – 12:00 PM & 5:00 PM – 6:00 PM.
• Only 1 attendant is permitted with a valid Attendant Pass per admitted patient.
• Outside food, flowers, and children below 10 years are strictly not allowed inside wards/ICU.
• Patient's Original Aadhaar Card + PAN Card (or Voter ID).
• Health Insurance TPA Card / Policy Copy OR Ayushman Bharat Golden Card + Ration Card.
• For Emergency Cashless Admission, submit documents at Counter 3 within 24 hours of admission.
